- Timing: End of Plan or Change of Circumstances Plan Review
- Purpose: Provide a thorough review of the participant’s current NDIS plan and supports, assess family needs (with an emphasis on caregiver burnout), and recommend adjustments to align with evolving goals. This service includes an in-depth review of the participant’s NDIS goals, suggesting updates as necessary for a new plan. With consultation, the Social Worker will review engagement with other supports which the participant is engaged with (eg. OT, Speech and Behavioral Therapists), collaborate with all providers. This can help provide consistent advocacy of the participants needs and supports and assist with short to long term well being.
- Session Structure:
- Face-to-Face or Virtual Assessment: 1.5 hours – Comprehensive review of the participant’s NDIS plan, family support, alignment with goals, and assessment of current services.
- Stakeholder Coordination: 1 hour – Communication with other providers to ensure consistent support recommendations and unified goals.
- Detailed Report, Goal Recommendations, and Advocacy Letter Preparation: 2 hours – Includes preparing a report with funding justifications, revised goals, and a letter of advocacy for continued and future funding.
- Total Estimated Hours: 4.5 hours
Deliverables: Comprehensive report with funding recommendations, updated goals, and advocacy letter supporting the participant’s current and future needs.
