- Timing: Following a Review or start of a New Plan
- Purpose: Ensure alignment and effective communication among the participant’s current and potential providers. This service includes meeting with the participant and family to review existing supports and advocate for best engagement within Opentrax and other service providers. This support can be provided across the participants engagement within Opentrax.
- Eg. support letter to advise of carer burnout, recommendations of engagement in OpentraX sessions 2x month to ensure participant goals are met, in addition to recommending extra funding for engagement with OT
- Session Structure:
- Face-to-Face or Virtual Meeting with Participant/Family: 1 hour – Discussion of current supports, provider roles, and participant goals.
- Provider Coordination: 1 hour – Contacting providers to ensure goal alignment and consistent service recommendations.
- Letter of Support/Progress: 1 hour – Document to update on current providers & service consistency, goals alignment and support needs.
- Total Estimated Hours: 3 hours
Deliverables: Letter of support for new providers (optional), ensuring consistency in participant care, along with a unified service approach.
