- Timing: End of Plan or Change of Circumstances Plan Review
- Purpose: Provide a thorough review of the participant’s current NDIS plan and supports, assess family needs (with an emphasis on caregiver burnout), and recommend adjustments to align with evolving goals. This service includes an in-depth review of the participant’s NDIS goals, suggesting updates as necessary for a new plan. The Social Worker will collaborate with other providers to ensure consistent advocacy and clear support recommendations for the participant’s long-term well-being.
- Session Structure:
- Face-to-Face or Virtual Assessment: 1.5 hours – Comprehensive review of the participant’s NDIS plan, family support, alignment with goals, and assessment of current services.
- Stakeholder Coordination: 1 hour – Communication with other providers to ensure consistent support recommendations and unified goals.
- Detailed Report, Goal Recommendations, and Advocacy Letter Preparation: 2 hours – Includes preparing a report with funding justifications, revised goals, and a letter of advocacy for continued and future funding.
- Total Estimated Hours: 4.5 hours
Deliverables: Comprehensive report with funding recommendations, updated goals, and advocacy letter supporting the participant’s current and future needs.
Family Education & NDIS Management

- Purpose: Educate families on effectively managing their NDIS plans, including advocacy strategies, how to request reviews, and navigating the NDIS for maximum support.
- Session Structure:
- Face-to-Face/Virtual Session: 1 hour (existing), 1.5 hour (new participants)
- Follow-Up Resources: 30 minutes (includes written resources on NDIS management and advocacy strategies)
- Total Estimated Hours: starting from 1.5-2 hours* open to review based on participants needs
- Deliverables: Comprehensive guide on NDIS management, family advocacy tips, and recommended support services.
Burnout Prevention & Family Counselling

Purpose:
This service aims to support family members in managing the long-term stressors associated with caregiving, helping prevent burnout and build resilience. By offering counselling and personalised tools, families gain the skills needed to care for both themselves and their loved ones effectively. The focus is on sustainable, proactive support to ensure families feel empowered and well-prepared in their caregiving roles.
Session Structure:
•Counselling Session: 1 hour (in-person or virtual) – A supportive session focused on recognising burnout signs, discussing caregiver challenges, and developing self-care strategies.
•Follow-Up Support: 30 minutes (optional, as needed) – Additional resources, including stress management tips and guidance on maintaining resilience, provided via email or virtual check-ins.
Total Estimated Hours: 1.5 hours

- Timing: Following a Review or start of a New Plan
- Purpose: Ensure alignment and effective communication among the participant’s current and potential providers. This service includes meeting with the participant and family to review existing supports and outline a unified plan, producing a letter of support to be used across both current and new providers within a 12-month period.
- Eg. support letter to advise of carer burnout, recommendations of engagement in opentrax sessions 2x month to ensure participant goals are met, in addition to recommending extra funding for engagement with OT
- Session Structure:
- Face-to-Face or Virtual Meeting with Participant/Family: 1 hour – Discussion of current supports, provider roles, and participant goals.
- Provider Coordination: 1 hour – Contacting providers to ensure goal alignment and consistent service recommendations.
- Letter of Support/Progress: 1 hour – Document to update on current providers & service consistency, goals alignment and support needs.
- Total Estimated Hours: 3 hours
Crisis Skills & Immediate Support

Purpose:
Designed to provide immediate, compassionate support to families facing crises, this service equips them with the tools and strategies needed to navigate unexpected challenges, manage stress, and strengthen resilience. Crisis counselling is ideal for families experiencing acute stressors related to caregiving or sudden changes in participant needs, allowing them to feel supported and confident in managing high-stress situations.
Session Structure:
•Counselling Session: 1 hour (in-person or virtual) – Immediate, solution-focused counselling to address the family’s current challenges, explore coping strategies, and develop an action plan for immediate concerns.
•Follow-Up Support: 30 minutes (as needed) – Optional ongoing support to provide additional resources, check in on progress, and answer any further questions.
Total Estimated Hours: 1.5 hours
